What This Looks Like: You'll work with a case manager (Jeremy, initially) who understands what you're going through. Together, you'll:

  • Find Housing: Application help, landlord advocacy, connections to LGBTQ+-friendly housing, emergency shelter navigation

  • Access Healthcare: Medicaid enrollment, finding gender-affirming care providers, mental health connections, medication continuity

  • Get IDs & Documents: Driver's licenses, state IDs, birth certificates, Social Security cards, name/gender marker changes

  • Navigate Benefits: Food assistance (SNAP/EBT), disability benefits, cash assistance, healthcare enrollment

  • Find Employment: Resume building, job search support, connections to LGBTQ+-friendly employers, vocational training

  • Handle Legal Issues: Court obligation navigation, legal aid referrals, record sealing/expungement information

  • Stay Safe: Crisis planning, harm reduction resources, connections to emergency support

How Often:

  • Weekly check-ins during your first month

  • Bi-weekly contact during months 2-3

  • Monthly support during months 4-6

  • Ongoing access whenever you need it

Service Area 2: Mutual Aid & Material Support

What We Provide:

  • Clothing: Vouchers for job interviews, court appearances, everyday wear

  • Transportation: Bus passes, gas cards, rideshare vouchers to get to appointments

  • Phones: Cell phones and minutes to stay connected

  • Hygiene Items: Toiletries, personal care products, menstrual products

  • Emergency Funds: Small grants for housing deposits, utilities, urgent needs

  • IDs & Documents: Fees for birth certificates, state IDs, legal filings

No strings attached. No requirements. Just support.

Service Area 3: Peer Community

What This Looks Like:

  • Support Groups: Monthly gatherings (virtual and in-person) facilitated by people with lived experience

  • Social Events: Community meals, movie nights, Pride celebrations—connection without agenda

  • Connections to LGBTQ+ Spaces: We'll introduce you to affirming organizations, events, and community spaces

  • Peer Mentorship: Matching with other formerly incarcerated LGBTQ+ people (if you want)

  • Advisory Council: Opportunities to shape OutTogether's programs and guide our work
